At the Diabetes Clinic, our Diabetes Educator and your GP work as a team to provide clear advice, practical tips, and encouragement to help you stay on track. With their expert knowledge, they can also guide you to other health professionals—like an optometrist, ophthalmologist or podiatrist—when extra care is needed, making sure you get the best support for your health.

  • people living with type 1 diabetes
  • people living with type 2 diabetes
  • people who have been diagnosed with prediabetes or told they have high blood glucose
  • Mothers after pregnancy who had gestational diabetes or those that are planning another pregnancy
  • people living with type 1 diabetes who are planning a pregnancy or have recently been pregnant
  • people living with an intellectual disability and type 1 or type 2 diabetes, and their carers or guardians

Diabetes & Chronic Condition Management Plan  

Diabetes is a complex condition, and people often need extra support to manage their health on a daily basis. The Diabetes Educator at the Diabetes Clinic can help you get extra help from health professionals like a dietitian, exercise physiologist, or podiatrist as part of your care team.

With a Diabetes Chronic Condition Management Plan (CCMP), you can get up to five visits each year to these professionals, and Medicare helps cover the cost.

People living with Type 2 Diabetes may also be eligible to attend up to eight additional group education programs each year delivered by exercise physiologists and dietitians.

What is the cost of a Diabetes Clinic?
